Please include the correct spelling and designation (i.e., MD, MPH, FAAP) for each individual nominated.
Brief description to their work/reason they should be nominated. It may be work on a new and innovative project in the past year or for their lifetime achievement.
Please do not nominate individuals from the previous year who received an award.
Nominated pediatricians must be a member of the AAP
Nominations must be received by December 31 to be considered for an award for that calendar year. Nominations received after this date will be considered the following year.
